What To Bring To Your Appointment

It is always our goal to make your visit with us as seamless as possible. For this reason, we have a simple list of items that you should bring with you each time you have an appointment at SkinSpire Dermatology and Aesthetics.

1. Medical Insurance ID card must be shown at every visit if present for a medical concern. This may seem like an inconvenience; however, this precaution will ensure that you do not receive a bill from us that should have been processed by your insurance. Due to the constant changes in medical insurance identification numbers and plan details, seeing your card on every visit enables us to bill your insurance properly so that you do not get billed as a self-pay patient.
2. Name and date of birth of the primary holder for your medical insurance.
3. A photo ID.
4. Any necessary data: current medications, both prescribed and over the counter, major medical history, and dermatological history.
5. Copay or payment for treatment.

• Any past due balances must be collected prior to your visit.
• IF THE PATIENT IS A MINOR, A PARENT OR LEGAL GUARDIAN MUST ATTEND THE VISIT.
